The problem.

At the age of 51, Mike struggled with diabetes, sleep apnea, high blood pressure and heart disease. He knew he needed to make a change in his lifestyle to improve his health now and into the future. 

Mike knew it was important to lose weight to lower his risk of long-term complications. He tried weight loss programs he’d seen online, but he found it hard to stick with them. So, he made an appointment with a First Stop Health diabetes educator for help. 

 

The solution.

First, the diabetes educator helped Mike make an appointment with a First Stop Health doctor to review his medical history and complete lab work. Then, she talked with him about his health goals, what was keeping him from achieving those goals and how health coaching can help.

Together they came up with a plan for a healthier lifestyle that worked for him. To make it easier to stick to the plan, Mike also met with a registered dietitian to talk about his eating habits and what changes he needed to make for weight loss.  

 

The result.

Through the support of his virtual care team, Mike has lost 70 pounds and his A1C dropped 1.3 points. He’s sleeping better, exercises regularly and has healthier eating habits. He meets regularly with his doctor and coaches for ongoing support to reach his long-term health goals.
