HEALTH COACHES

Health coaching is a confidential, non-judgmental and motivational partnership for you to achieve the best version of yourself. Each visit is centered around creating a vision for your life and collaborating to design attainable goals. Your coach will support you through accountability, education, resources and building a relationship. 

Health coaches support you in whatever way is best! We usually recommend starting with visits once a week for 4-6 weeks and then working with your coach to decide the best timing for continued visits to meet your goals. 

Each visit is 20 minutes.

Health coaches are board certified experts in human behavior, motivation, health and wellness. They are educated on various techniques and health topics such as exercise, nutrition, sleep, brain health, work/life balance and much more. 

There are many reasons to see a health coach! You might have specific goals you want to work on in mind, or your health coach can help you come up with new goals. The top reasons to see a health coach are: 

  • Healthy mom & baby
  • Improve heart health
  • Improve sleep
  • Quit tobacco
  • Life satisfaction
  • Managing stress 

Plan to be in a quiet and private space when joining the video call for the visit. If you have any goals or questions you want to talk about, try writing them down so you don’t forget. You may also want to keep paper and a pen near you so you can take notes if you would like. 

Yes, other team members are available to support your goals as needs are identified. You can see a First Stop Health primary care doctor, urgent care doctor, dietitian and diabetes educator whenever you choose. Your health coach can also help you determine which other doctors or specialists would be most helpful to you. 

DIETITIANS

Visits are focused on guiding your healthy eating and lifestyle goals through nutrition education and support. Your dietitian will help design a plan to support you in achieving your nutrition goals. They can also identify ways to eat healthy that fit your lifestyle and daily routine and provide practical tips and recipes for meal planning. 

Visits can be scheduled as often as needed depending on your immediate concerns and needs. A good plan is weekly or every two weeks until you meet your goal. 

Visits are 40 minutes long.

A registered dietitian is a board-certified expert in food and nutrition.  

The top reasons to see a dietitian are: 

  • Manage weight
  • Improve eating habits
  • Meal planning tips
  • Specific nutrition guidance
  • Emotional or disordered eating

DIABETES EDUCATORS

These visits help prevent developing diabetes or controlling existing diabetesVisits can include education on nutrition, answering any questions, and information on medications, glucose monitoring and self-care. You can be supported through setting short- and long-term goals to live a healthy life and prevent long-term complications.  

Visits can be scheduled as often as needed depending on your immediate concerns and needs. A good plan is weekly or every 2 weeks until you meet your clinical goal. 

Visits are 40 minutes long.

Diabetes educators hold an advanced degree and certification through the National Board of Diabetes Educators. They are health professionals who possess comprehensive knowledge of and experience in diabetes prevention and diabetes management.  

Topics of conversation may include:  

  • Eating healthy
  • Understanding and managing my medications
  • Exercising
  • Glucose monitoring and testing 
  • Support for high and low blood sugar
  • Reducing risk for complications
  • Diabetes care with pregnancy 

Plan to be in a quiet and private space when joining the video call for the visit. Feel free to bring any information you think will be relevant, along with your medical and diabetes history. You may also want to have paper and a pen ready in case you want to take notes.
